On your first visit you will have to complete a medical and dental history for our office.  Feel free to print  the New Patient Registration Form and bring with you along with your insurance information to your appointment.  Also you will find a copy of our privacy notice.  Once you are a patient of our office and you have any changes in medical condition you may print the update.
